As primary care providers, we are the first point of contact for individuals in their acute medical care, management of chronic medical conditions, and preventive health care. The role of the primary care provider is to oversee and coordinate care and to help patients navigate the health care system. We provide general primary care for pediatrics, adolescents and adults. We reserve same-day visits for acute medical problems in addition to providing ongoing management for chronic conditions. We also offer compassionate care for depression and anxiety disorders. We refer to appropriate specialists when needed, and help to oversee patients’ care in close communication with other providers. Our focus is on comprehensive primary care with a collaborative approach tailored to each unique person.

Preventive care is a central part of a person’s overall health, that is why we provide a comprehensive annual visit as a time to address prevention and wellness. In addition to a thorough physical examination, we perform cancer, osteoporosis and blood work screenings as well as counsel patients regarding vaccine recommendations. We also administer immunizations appropriate for each person’s age and medical conditions. In addition, we focus on maintaining a healthy relationship with food and physical activity. Using a thorough and holistic approach, our providers engage in shared decision-making with each individual regarding their personalized preventive care plan.

A Medicare annual wellness visit is a free annual visit for those 65 and older who have Medicare. The focus of this visit is on health prevention and consist of:

1.       A health risk assessment

2.       A review of your medical, surgical and family history

3.       A review of other providers on your healthcare team

4.       Routine measurements such as height, weight, blood pressure and a visual acuity screening

5.       Cognition screening

6.       Reviewing risk factors for depression or other mood disorders

We will discuss an appropriate screening schedule and provide an individualized preventative health care plan that includes health advice and referrals to health education or mental health resources  as well as provide health education and preventative counseling.

Medicare Annual Wellness visits do not include:

1.       A physical exam

2.       Advanced care planning

3.       Addressing other medical problems

Eating disorders are common conditions which often go undiagnosed or undertreated in routine medical care. In addition to anorexia nervosa, bulimia nervosa, and binge eating disorder, there is a continuum of disordered eating that can involve an unhealthy relationship with food and physical activity. We offer a safe, non-judgmental environment where patients can seek medical care for eating disorders. We collaborate closely with dietitians, therapists, and psychiatrists in the community to provide a collaborative, team-based approach. We also work with hospitals and eating disorder treatment centers when a higher level of care is required.  It can take courage to seek help for an eating disorder, but individuals can begin the process of recovery with the help of an experienced and caring team of providers.

We are happy to offer telehealth sessions to our established patients as our providers see fit.
